What is the MSRA?

The Multi-Specialty Recruitment Assessment is a computer-based assessment used in recruitment to a range of UK medical specialty training programmes. It tests essential competences through clinical scenarios and is delivered as two independently timed papers.

One assessment. Two kinds of judgement.

The MSRA is designed to assess how candidates apply knowledge and professional judgement—not simply how many facts they can recall. The exact role of the score varies by specialty and recruitment round, so candidates should always check the current guidance for the programme to which they are applying.

Current official guidance describes a 170-minute assessment: 95 minutes for Professional Dilemmas and 75 minutes for Clinical Problem Solving.

Professional Dilemmas

Situational judgement in areas such as patient safety, communication, teamwork, probity, prioritisation and appropriate escalation. Strong answers reflect safe, proportionate and professional action.

CPS

Clinical Problem Solving

Applied clinical knowledge across investigation, diagnosis, treatment and management. The focus is selecting the most appropriate next step from realistic clinical information.

Apply knowledge safely

Translate clinical information into the most appropriate action.

Exercise judgement

Balance professionalism, patient safety and practical constraints.

Prioritise effectively

Recognise urgency, red flags and the correct order of action.

Reason under time

Make defensible decisions efficiently in an exam setting.

How to prepare for the MSRA

Build accuracy first, then pace. Use every question to understand the decision behind the answer and turn your results into a focused revision plan.

Know the format

Read the current official guidance and the instructions for both papers before building a study plan.

Set a baseline

Complete a mixed set without over-preparing. Use the result to identify weaker domains and slow decisions.

Build breadth

Revise common presentations, investigations and management while practising professional judgement regularly.

Review every option

Study why the best answer works and why each alternative is less appropriate. Record recurring errors.

Practise under time

Move from targeted sets to timed mixed practice, then revisit wrong answers and under-sampled areas.

A useful weekly rhythm: targeted revision → timed questions → full rationale review → analysis of accuracy, pace and coverage → a revised plan for the next week.

Dr Blessing Dziwa is a physician with more than 20 years of international clinical experience. He served for 10 years as a Peace Corps Medical Officer and currently works as a Registrar in Acute and General Medicine.

His clinical work centres on the assessment and management of acutely unwell patients, where decisions are often complex, time-critical and ethically challenging. This practical experience shapes AMT’s emphasis on clear reasoning, patient safety and proportionate professional judgement.

He is also an instructor for IMPACT — Ill Medical Patients’ Acute Care and Treatment and occasionally serves as a course lead, supporting clinicians who manage acute medical admissions.
